Introduction

Welcome to the Inera Core Implementation Guide - the Swedish national FHIR profiles for healthcare information exchange.

This Implementation Guide (IG) defines the foundational FHIR profiles, extensions, and terminology bindings required for interoperability in Swedish healthcare. It serves as the core specification for the Nationell Patientöversikt (NPÖ) and other national healthcare information exchange initiatives in Sweden.

Purpose

The Inera Core IG provides:

  • Swedish Base Profiles: National adaptations of FHIR resources aligned with International Patient Access (IPA) principles
  • TKB Integration: Complete mappings from TKB (Tjänstekontraktsbaserade tjänster) logical models to FHIR resources
  • Swedish Terminology: Comprehensive bindings to Swedish code systems including:
    • Swedish national identifiers (personnummer, samordningsnummer)
    • HSA-ID for healthcare professionals and organizations
    • Swedish clinical code systems (KV Befattning, KV Vårdkontakttyp, NPL, ATC, ICD-10-SE, etc.)
  • PDL Compliance: Support for Patientdatalagen (Patient Data Law) requirements including provenance tracking and access control

Scope

This IG covers the following areas:

Patient and Demographics

  • Patient identification using Swedish national identifiers
  • Gender and demographic information aligned with Swedish standards

Clinical Content

  • Diagnoses (ICD-10-SE, SNOMED CT SE)
  • Allergies and intolerances (NPL, ATC coded)
  • Medications (NPL, ATC coded)
  • Laboratory results (NPU, LOINC coded)
  • Clinical observations and assessments

Healthcare Providers

  • Practitioner identification via HSA-ID
  • Professional roles (KV Befattning)
  • Organizational hierarchy (vårdgivare → vårdenhet → organisationsenhet)

Care Activities

  • Encounters/care contacts (vårdkontakter)
  • Care documentation
  • Provenance and audit trails for PDL compliance
